PSA/ NSO DOCUMENTS

       If you need a Birth Certificate, Marriage Certificate, Death Certificate or CENOMAR (Certificate of No Marriage Record) from the Philippine Statistics Authority (PSA), formerly National Statistics Office (NSO), for delivery anywhere in the Philippines or abroad, click on this link www.psaserbilis.com.ph.

 

REQUIREMENTS AND PROCEDURES IN APPLYING FOR DELAYED REGISTRATION OF BIRTH, MARRIAGE OR DEATH IN THE PHILIPPINES. 

 

Effective Wednesday, 18 March 2020, all consular services are suspended until further notice.   The premises of the Philippine Consulate General will be closed.

 

  PASSPORT FORMS

     IMPORTANT NOTICE:

         Effective 12 March 2020 until July 2020 (or until the COVID-19 situation improves or stabilizes), the releasing of passports will be done by mail only.   Personal pickup of new passports will not be allowed in order to minimize the risk of COVID-19 infection to the public.

 

         Applicants should ensure that they can monitor or track their mailing envelopes in order to avoid the loss of their new passport.

 

DUAL CITIZENSHIP

 

IMPORTANT NOTICE:

             Effective Monday, 16 March 2020, the Consulate will temporarily suspend the processing of dual citizenship applications.   Likewise, the oath taking ceremony for dual citizens is hereby suspended effective 13 March 2020, in order to minimize the risk of COVID-19 infection to the public.   A new schedule will be announced accordingly, once the situation has improved or stabilized.

   DOCUMENTATION

 

   CIVIL REGISTRY

      For Civil Registry applications (REPORT OF MARRIAGE, REPORT OF BIRTH and REPORT OF DEATH), FORMS MUST BE TYPEWRITTEN.   Handwritten forms will not be accepted.   You can fill-up the forms online before printing.

IMPORTANT NOTICE:

       Effective 12 March 2020 until July 2020 (or until the COVID-19 situation improves or stabilizes), all civil registry applicants are highly encouraged to send their applications for Reports of Birth, Marriage and Death by mail in order to minimize the risk of COVID-19 infection to the public.
