Passport


The Philippine Consulate General processes applications for passport. They are transmitted to the Department of Foreign Affairs in Manila for printing. Please allow two (2) months processing time from the date of application up to the date of release of the new passport to the applicant. The Philippine Passport is at all times the property of the Republic of the Philippines. It must be surrendered upon demand by an authorized representative of the Philippine Government. Alteration, addition, or destruction of contents is strictly prohibited. Any unauthorized change will render a passport invalid and will have accompanying penalties. Using the passport as collateral is against the law and will automatically render the passport cancelled. The information provided in the passport application form must be true and correct. Applicants should bring complete requirements, which should be authentic. Under the law, making false statements in the application form, furnishing falsified or forged documents in support thereof are punishable by law. Also, the Consulate reserves the right to request additional requirements, if necessary.

Important Reminders:

    PERSONAL APPEARANCE IS REQUIRED at all times and application is by appointment only.

    For application at the Consulate, please secure an appointment via the Department’s Online Appointment System by accessing the following link: https://www.passport.gov.ph. Applicants are advised to be at the Consulate’s premises not earlier than 10 minutes of their appointment schedule.

    Photos will be taken at the Consulate (Do not bring your own photos).

    For photo capturing, applicants are required to wear a decent attire. Jewelry, piercings, glasses and colored contact lenses will not be allowed.

    Bring photocopies as required. The Consulate does not offer photocopy services.

    Bring your own pen and observe applicable health protocols.

    For the delivery of new passports, each applicant must BRING a self-addressed envelope (preferably USPS Priority Mail or UPS Prepaid envelope, with stamps and tracking number) except FEDEX.

Requirements

  • 1. Print Passport Application form attached on your appointment confirmation.
  • 2. Original Current Passport and photocopy of data page (and photo page for old brown passport)
  • 3. Original and photocopy of green card or US visa
  • 4. For Dual Citizens
    1. RA 9225- original and photocopy of Identification Certificate, Oath of Allegiance, Order of Approval, or Certificate of Retention/Re-acquisition of Philippine Citizenship or any equivalent document issued by the Bureau of Immigration
    2. By Birth - original and photocopy of PSA-issued Birth Certificate or PSA-issued Report of Birth

  • 4. Notarized Special Power of Attorney (SPA) executed by either parent if married (notarized at a PHL Embassy/ Consulate or apostillized) authorizing the adult companion (with a copy of the parent’s ID/passport as attachment), or executed by the mother if unmarried with the father (with a copy of the mother’s ID/passport as attachment)
  • 5. Original and photocopy of passports / valid IDs of both parents, guardian or authorized adult companion

      2. • Original and photocopy of PSA-issued Birth Certificate or Report of Birth
    2. For annulled marriages or divorced applicants:
      1. • Original and photocopy of PSA-issued Marriage Certificate or Report of Marriage (ROM) with annotation reflecting the nullity or dissolution of marriage
      2. • Original and photocopy of PSA-issued Certificate of Live Birth or Report of Birth

  • The following passport applicants may be accommodated as walk-ins (even without an appointment) on work/week days (except Fridays and holidays):
  • 1. Applicants who are 60 years old and above;
  • 2. Minors who are 7 years old and below (with an adult companion);
  • 3. Applicants with disability (must present a Disability ID);
  • 4. Pregnant applicants (must present medical records); and,
  • 5. Applicants with an immediate need of a passport (must present proof of urgency, such as a death certificate of a family member, an expiring stay/work permit and the like).

  • For walk-ins, please use the appropriate Application Form in the “Downloadable Forms.” Accomplish and bring the same when you proceed to the Consulate.

    The Consulate General, during the processing of applications, will prioritize applicants with confirmed appointments.

  • — First-time Applicant: $60

  • — Lost Passport
    1. A) ePassport: $150
    2. B) Non-ePassport: $90
  • — Affidavit of Loss: $25
  • A travel document is an identification document issued in lieu of a passport and valid for one-way, direct travel to the Philippines. It may not be used for travelling to other destinations. A travel document is issued to a Filipino citizen who needs to urgently travel home at the time of an emergency.

    The Consulate General only accepts applications for Travel Document from Filipinos based in the following States: ILLINOIS, INDIANA, IOWA, KANSAS, MICHIGAN, MINNESOTA, MISSOURI, NEBRASKA, NORTH DAKOTA, OHIO, SOUTH DAKOTA AND WISCONSIN. Send the complete requirements only by mail. No walk-ins. Incomplete requirements will not be processed and will be returned to the sender. Additional requirements may be requested.

    REQUIREMENTS AND PROCEDURE
  • 1. Duly accomplished Travel Document Application form, filled-out PDF or handwritten in black or blue ink. Indicate contact details: mobile number and email address. Parents should accomplish the form for minors.
  • 2. Notarized Affidavit on Travel Document Application
  • 3. Original Philippine passport (and photocopy of data page). If lost passport, submit the following:
    1. • Original and photocopy of PSA-issued Birth Certificate/ Report of Birth
    2. • Original and photocopy of PSA-issued Marriage Contract/ Report of Marriage (for married women using their husbands’ last name)
    3. • Notarized Affidavit of Loss
    4. • Police Report (if the lost passport is still valid)
    5. • Photocopy of a valid ID (State ID, Driver’s license, School or Work ID)
  • 4. Four (4) identical colored photos (2x2") with plain white background taken within the last six (6) months printed in photo paper. The photo must show applicant in proper attire without eyeglasses. Blurred or low quality photos will not be accepted.
  • 5. Photocopy of green card or US visa
  • 6. Original notarized Affidavit of Consent executed by both parents (for minors)
  • 7. Copy of airline ticket/itinerary of travel to the Philippines. The date of travel should not be beyond one (1) month from the date of application.(NOTE: Before purchasing an air ticket, check with the airline if they will honor the Travel Document in lieu of a passport)
